For applications for the Disability Health Board Report for Adults, an appointment should be made by calling 0224 552 05 90 during working hours (08:00/17:00). On the day and time of the appointment set for the report application, the patient must personally come to the "Disability and SOLGER Health Board" unit with the Republic of Turkey Identity Card. The applications of patients who do not have their Republic of Turkey Identity Card with them will not be accepted.

As the patient will be photographed, he/she must be present at the application.

If the patient is unable to carry out the procedures, the first-degree relative must come with the Republic of Turkey Identity Card. The relative of the patient who does not have the Republic of Turkey Identity Card will not be able to carry out the report procedures related to the patient.

As a result of the objection to the previously created Disability Health Board Report for Adults, when the referral letter of the Provincial Health Directorate reaches our Centre Directorate, the persons will be called by us and an application appointment will be made.

Is there a charge for the Disability Health Board Report for adults?

Disability Health Board Report for Adults is charged if the report result is "0%". However, if the report result is 1% and above, the report fee will not be charged.
