Disability and Resolution Health Board Procedures

Disability Health Board Report for Adults: It is a document prepared by the Disability Health Board stating the disability and health status of the persons.

The Disability Health Board Report for Adults can be used for the following purposes:

• Disabled identity card
• Home care fee
• Benefit from social care entitlements
• Getting a job as a disabled person
• Tax reduction (insured employees, disabled pension)
• Payment of salary in accordance with Law No. 2022
• To benefit from the SCT discount
• Hearing aids, orthotics, prostheses, wheelchairs, etc.

For applications for the Disability Health Board Report for Adults, an appointment should be made by calling 0224 552 05 90 during working hours (08:00/17:00). On the day and time of the appointment set for the report application, the patient must personally come to the "Disability and SOLGER Health Board" unit with the Republic of Turkey Identity Card. The applications of patients who do not have their Republic of Turkey Identity Card with them will not be accepted.

As the patient will be photographed, he/she must be present at the application.

If the patient is unable to carry out the procedures, the first-degree relative must come with the Republic of Turkey Identity Card. The relative of the patient who does not have the Republic of Turkey Identity Card will not be able to carry out the report procedures related to the patient.

The documents that must be brought for the application for the Disability Health Board Report for Adults are as follows:

• If the patient has an old report, a photocopy of the old report must be brought.
• If the patient has a guardian, a photocopy of the decision appointing a guardian must be brought.
• If there are medical documents regarding their diseases [epicrisis reports, pathology report, MRI (MRI) reports, printout of the medications used in the last year (from pharmacy), etc.] must be brought.
• The personnel who will carry out the procedures for patients coming from the institution (nursing homes) must come with the assignment letter and the Republic of Turkey Identity Card. The personnel who do not have the assignment letter and Republic of Turkey Identity Card will not be able to carry out the report procedures related to the patients.
• Patients who come with a referral from official institutions [Social Security Institution (SGK), Tax Office, Social Services Directorate, Police Department, District Governorships] must come with the referral letter written to Bursa Uludağ University Health Application and Research Centre Directorate. Applications of patients who do not have the referral letter with them will not be accepted.

As a result of the objection to the previously created Disability Health Board Report for Adults, when the referral letter of the Provincial Health Directorate reaches our Centre Directorate, the persons will be called by us and an application appointment will be made.

Disability Health Board Report for Adults is charged if the report result is "0%". However, if the report result is 1% and above, the report fee will not be charged.

The report fee determined according to the Public Health Services Sales Tariff Procedures and Principles will be requested from the person for the reports that result in "0%". Persons applying for the Disability Health Board Report for Adults will be informed by signing the "Disability Health Board Report Pricing Information and Consent Form".

Special Needs Report for Children (ÇÖZGER): It is a document prepared by the Çözger Health Board stating the special needs of the child.

The Special Needs Report for Children (SOLGER) can be used for the following purposes

• To receive special training
• Disabled identity card
• Home care fee
• Tax relief
• Benefit from social care entitlements
• Payment of salary in accordance with Law No. 2022
• To benefit from Ötv discount
• Hearing aids, orthotics, prostheses, wheelchairs, etc.

For Special Needs Report for Children (ÇÖZGER) applications, an appointment should be made by calling 0224 552 05 90 during working hours (08:00/17:00). On the day and time of the appointment for the report application, the patient must personally come to the "Disability and SOLGER Health Board" unit with the Republic of Turkey Identity Card. The applications of patients who do not have their Republic of Turkey Identity Card with them will not be accepted.

• As the patient will be photographed, he/she must be present at the application.
• For the application procedures of pediatric patients under 18 years of age, a first-degree relative (guardian, if any) must accompany the pediatric patients.

The patient must come with his/her Republic of Turkey Identity Card. The relatives of the patient who do not have their Republic of Turkey Identity Card with them will not be able to carry out the report procedures related to the patient.

The documents that must be submitted for the Special Needs Report for Children (ÇÖZGER) application are as follows

• If the patient has an old report, a photocopy of the old report must be brought.
• If the patient has a guardian, a photocopy of the decision appointing a guardian, and if the parents are separated, a photocopy of the custody document.
• If there are medical documents regarding their diseases [epicrisis reports, pathology report, MRI (MRI) reports, printout of the medications used in the last year (from pharmacy), etc.] must be brought.
• The personnel who will carry out the procedures for patients coming from the institution (nursing homes) must come with the assignment letter and the Republic of Turkey Identity Card. The personnel who do not have the assignment letter and Republic of Turkey Identity Card will not be able to carry out the report procedures related to the patients.
• Patients who come with a referral from official institutions [Social Security Institution (SGK), Tax Office, Social Services Directorate, Police Department, District Governorships] must come with the referral letter written to Bursa Uludağ University Health Application and Research Centre Directorate. Applications of patients who do not have the referral letter with them will not be accepted.

As a result of the objection to the previously created Special Needs Report for Children, when the referral letter of the Provincial Health Directorate reaches our Centre Directorate, the persons will be called by us and an application appointment will be made.

The result of the Special Needs Report for Children '' If the report comes out as "No Special Needs", it is charged. However, the report fee will not be charged for the reports that are decided to have special needs as a result of the report.

The report fee determined according to the Public Health Services Sales Tariff Procedures and Principles for the reports that are issued as "No Special Needs" will be requested from the first degree relative (guardian, if any) if the person on whose behalf the report is issued is a child patient under the age of 18. The first-degree relative (guardian, if any) of the patient applying for the Special Needs Report for Children will be informed by signing the "Disability Health Board Report Pricing Information and Consent Form".

Health Board Report for Adults Reporting Status Due to Terrorism, Accident and Injury: It is a report issued by the Disability Health Board for terrorism, accidents and injuries, indicating the rate of terrorism and accidents.

The Medical Board Report for Adults Declaring Conditions Due to Terrorism, Accidents and Injuries can be used for purposes such as obtaining compensation from the insurance company.

An appointment should be made by calling 0224 552 552 05 90 during working hours (08:00/17:00) for the applications for the Health Board Report Report for Adults Reporting Status Due to Terrorism, Accident and Injury. On the day and time of the appointment for the report application, the patient must personally come to the "Disability and SOLGER Health Board" unit with his/her Republic of Turkey Identity Card. Applications of patients who do not have their Republic of Turkey Identity Card with them will not be accepted.

• As the patient will be photographed, he/she must be present at the application.
• If the patient is unable to carry out the procedures, the first-degree relative must come with the Republic of Turkey Identity Card. The relative of the patient who does not have the Republic of Turkey Identity Card will not be able to carry out the report procedures related to the patient.

• If the patient has a guardian, a photocopy of the decision appointing a guardian must be brought.
• If the first hospital the person goes to after the accident and injury is a State Hospital, an epicrisis report must be brought, and if it is a University Hospital, a forensic report or anamnesis records must be brought.
• If there are medical documents regarding their diseases [epicrisis reports, pathology report, MRI (MRI) reports, printout of the medications used in the last year (from pharmacy), etc.] must be brought.
• The accident report issued after the accident and injury must be submitted.
• For traffic accidents, an official letter from the insurance company or the court must be brought to Bursa Uludağ University Health Application and Research Centre Directorate.
• For work accidents, it is absolutely necessary to bring the official letter written from the court to Bursa Uludağ University Health Application and Research Centre Directorate.

Medical Board Reports for Adults for Terrorism, Accidents and Injuries are chargeable. The report fee determined according to the Public Health Services Sales Tariff Procedures and Principles will be requested from the person. Persons applying for the Medical Board Report for Adults Reporting Terrorism, Accidents and Injuries will be informed by signing the "Disabled Health Board Report Pricing Information and Consent Form".

Medical Board Report on Terrorism, Accidents and Injuries for Children: This is the report issued by the ÇÖZGER Health Board for terrorism, accidents and injuries, indicating the level of special needs related to terrorism and accidents.

The Medical Board Report for Children Reporting Conditions Due to Terrorism, Accidents and Injuries can be used for purposes such as obtaining compensation from the insurance company.

An appointment should be made by calling 0224 552 552 05 90 during working hours (08:00/17:00) for the applications for the Health Board Report Report Report for Children Due to Terrorism, Accidents and Injuries. On the day and time of the appointment for the report application, the patient must personally come to the "Disability and SOLGER Health Board" unit with his/her Republic of Turkey Identity Card.
